The following checklist is for reference purposes only. It is not a complete list of documents needed to complete your tax return. If you are unsure whether or not to include paperwork not listed below, always contact our office to confirm.

Required paperwork

Personal Information (for yourself and your spouse)

☐ A copy of your prior year tax return (if you have not filed in previous years, please provide the last return you filed)

☐ Copy of driver’s or non-driver’s license(s)

☐ Copy of social security card(s)

☐ Information about your stimulus payment, also known as economic impact payment (EIP) (include the IRS Notice 1444 if you have it)

☐ Bank account routing and account number to receive your refund by direct deposit or pay your balance due

☐ Traditional IRA basis (the amounts you contributed to the IRA that where already taxed)

☐ Social security/RRB income (SSA-1099, RRB-1099)

☐ Interest, dividend income (1099-INT, 1099-IRD, 1099-DIV)

☐ Income from the sales of stock or other property (1099-B, 1099-S)

☐ Purchased date and price of property you sold (if basis is not reported on 1099-B)

☐ Heath Savings Account (HSA) and long-term care reimbursements (1099-SA or 1099-LTC)

☐ Transactions involving cryptocurrency (Virtual currency)

☐ Gambling income (W-2G or records showing income and expenses)

☐ Jury duty income

☐ Hobby income and expenses

☐ Records and receipts paid for childcare expenses

☐ Form 1098-T from educational institutions

☐ Receipts for qualified educational expenses paid

☐ Form 1098-E for student loan interest paid

☐ Receipts for classroom expenses K-12 Educators and teachers

☐ Amounts paid to state and local income or sales tax, other than wages withholding

☐ Receipt for vehicle sales tax paid and / or personal property tax

☐ HSA and IRA contributions on form 5498-SA, 5498-QA, 5498-ESA

☐ Paperwork for federally declared disasters including; location of property, records to support property losses, receipt and records of rebuilding/repair costs, reimbursements of insurance and claims to be paid, and FEMA assistances
